Statement of Accuracy 

By submitting enrollment, you agree to all terms contained within these terms. You certify that the information contained in the enrollment application is true and correct. You understand that any misrepresentation or omission of information will be sufficient for rejecting this application or dismissal from Custom Canine Unlimited, LLC. If enrolled, you agree to abide by Custom Canine Unlimited, LLC rules and regulations outlined in the Custom Canine Unlimited Catalog. 

Terms

Custom Canine Unlimited, LLC, does not discriminate based on race, color, creed, age, national origin, disability, marital status, or status as a disabled veteran or Vietnam Era veteran in the recruitment of students, the recruitment and employment of faculty or staff, or the operation of any of its programs and activities, as specified in federal and state laws and regulations. Custom Canine Unlimited, LLC does not knowingly admit individuals in the United States illegally.

TERMS OF ENROLLMENT 

Warning

Under Georgia law, there is no liability for an injury or death of an individual entering a Custom Canine Unlimited, LLC premises, or training with Custom Canine Unlimited, LLC, if such injury or death results from the inherent risks of contracting COVID-19. You are assuming this risk by entering these premises or attending the training.

You understand and agree that Custom Canine Unlimited, LLC shall not be liable for any injury or damage to any person, animal, or property which results from your behavior or any canine’s behavior or the behavior of any person or animals attending any course with Custom Canine Unlimited, LLC. You also understand and agree that Custom Canine Unlimited, LLC, and its employees shall not be liable for any cost, expense, injury, or damage to you, your canine, or property resulting from your participation in any course. 

You understand that for any canine to participate in any training, they must be current on their vaccinations. You know that it is still possible for vaccinated dogs to become ill with an infectious condition despite being vaccinated. Additionally, You understand that this is not due to any circumstance or state in the Custom Canine Unlimited, LLC property, and agree that you will not hold Custom Canine Unlimited, LLC or its employees liable in the event my canine becomes ill with any infectious condition during or after a course. 

TERMS OF PERSONNEL TRAINING

Custom Canine Unlimited, LLC agrees to provide training based on the prescribed curriculum. You are aware that payment does not guarantee personnel completion or passing results. Your sole responsibility is to complete and pass each specific training course based on each course(s) set requirements. Custom Canine Unlimited may follow prescribed disciplinary avenues, as stated in the issued catalog, for the following reason, but are not limited to these reasons: 

a. Continued training of problematic Personnel or Personnel unable to complete each training course’s standards at the sole discretion of Custom Canine Unlimited, LLC. 

b. Custom Canine Unlimited, LLC reserve the right to expel Personnel attending training based on specific conditions resulting from specific training personnel conditions, such as; 

i. Medical Condition(s) 

ii. Unwillingness to complete the course 

iii. Overt safety concern(s) 

iv. Unresolvable or Unavoidable Personal Issues 

c. Personnel expelled from the training course does not void my financial responsibility to Custom Canine Unlimited, LLC. Custom Canine Unlimited will refund monies as prescribed in Custom Canine Unlimited Catalog. 

LODGING: 

Tuition fees are separate from Lodging fees.

EQUIPMENT/SUPPLIES: 

No equipment or supplies are included in tuition fees unless expressly written.

ACKNOWLEDGEMENTS: 

You fully understand that canine(s) is a Live Animal that must continue training to maintain a high level of proficiency. You fully understand that canine(s) can make their own decisions. Having and developing its thoughts can teach new behaviors and reactions based on the environment, situation, training, and instinct. Custom Canine Unlimited, LLC takes no responsibility for this. 

INDEMNIFICATION: 

For good and valuable consideration, the receipt and sufficiency of which are at this moment acknowledged, you now expressly agree to indemnify and hold harmless Custom Canine Unlimited, LLC, its successors, or assigns against all suits, actions, claims, demands, or damages, to the extent permitted by law, that arises from the following: 

d. Any injuries arising during all training, exercises, or event associated with Custom Canine Unlimited, LLC.
e. Any Canine or Handler Actions
f. The canine’s or Handler’s Training
g. Use of any Canine
h. Your Actions
i. Your Training
j. Your use of the dog (s) or any lack thereof on this list. 

GOVERNING LAW: 

This Agreement shall be governed by and construed by the laws of the State of Georgia. 

CUMULATIVE RIGHTS: 

All rights under this Agreement are cumulative and shall not be construed as exclusive unless otherwise required by law. 

WAIVER: 

The failure of either party to enforce any provisions of this Agreement shall not deem a waiver or limitation of that party’s right to subsequently enforce and compel strict compliance with every provision of this Agreement. 

IN WITNESS of which, the parties have caused this Agreement to be executed the day and year course enrollment is submitted. 

FINANCIAL ACKNOWLEDGEMENT 

As a student of Custom Canine Unlimited, LLC, who is voluntarily attending a course or training, You understand that certain expectations exist concerning my conduct within this course. By accepting enrollment in this course, You agree to abide by those conduct standards outlined in the Custom Canine Unlimited, LLC catalog. 

Should your private or public conduct result in my suspension by Custom Canine Unlimited, LLC, you understand you will forfeit all monies paid to Custom Canine Unlimited for the enrolled course. Any payments directly received by Custom Canine Unlimited, LLC from Veterans Affairs will be refunded by Custom Canine Unlimited, LLC, directly to Veterans Affairs at the pre-determined rate. You will be fully responsible for reimbursement to Veteran Affairs for any monies paid. 

You fully understand that you are personally responsible for all fees and tuition incurred by your attendance in an approved Veteran Affairs Education Benefits Course offered by Custom Canine Unlimited, LLC until all cost(s) are fully satisfied. 

You voluntarily wish to enroll in the selected approved Veteran Affairs Education Benefits Course chosen. You are setting enrollment in this course with an understanding of your financial responsibility toward Custom Canine Unlimited, LLC. 

You fully understand that Custom Canine Unlimited, LLC has no bearing on the approval or disapproval of any U.S. Department of Veteran Affairs Benefits. Only the U.S. Department of Veteran Affairs may approve or disapprove benefit payments. 

You agree to bear all financial responsibility to Custom Canine Unlimited, LLC.
